Reminder of mandatory daily symptom monitoring

COVID-19 Response and Recovery Task Force

Dear Bruin Community:

As a reminder, all individuals living, learning or working on UCLA properties (both vaccinated and unvaccinated) must complete the UCLA COVID-19 Symptom Monitoring and Vaccination Verification System Survey  before arriving on property each day.

For workers in UCLA Health hospitals and clinics please refer to the weekly COVID-19 all-staff email from UCLA Health

  • Students: Students who are attending class in person on campus must complete the survey prior to arriving to class.
  • On-Campus Residents: If an individual is living in a UCLA housing facility, they must complete the survey every morning.
  • Fully Remote Employees: Employees who are working fully remotely must update their status one time within the system to indicate they are completely remote as soon as possible.
  • Hybrid Employees: Employees who are working a hybrid in-person schedule must complete the survey prior to arriving on UCLA property on days when they will be working in person.

Verify your clearance status on the UCLA COVID-19 Clearance Portal

All UCLA students, faculty and staff can view their symptom monitoring clearance status on the UCLA COVID-19 Clearance Portal. Individuals may be asked to present their email certificate or log in to the portal as proof of completion when necessary. Bruins can also conveniently access the UCLA COVID-19 Symptom Monitoring and Vaccination Verification System, the UCLA COVID-19 Clearance Portal and their UCLA COVID-19 Clearance Certificate on the UCLA Mobile App. Download the app at one of the following links:

Supervisors/Department Chairs
Supervisors and department chairs are required to verify staff and faculty clearance through the UCLA COVID-19 Clearance Portal daily.

Instructors
Instructors of record for each class section are able to verify the clearance status of students enrolled in their section through the UCLA COVID-19 Clearance Portal. If students are not cleared or no survey was found, they should not be attending class in person.

Event, Store and Building Managers
Event, store and building managers may ask for the symptom monitoring clearance certificate from UCLA affiliates before allowing them into a building or an event when they cannot directly access that information.

UCLA COVID-19 Clearance Portal Updated Features
In addition to showing clearance status based on the daily symptom monitoring, the UCLA COVID-19 Clearance Portal has been updated to also show compliance status with the UC SARS-CoV2 (COVID-19) Vaccination Policy (PDF) for all students, faculty and staff. Individuals will be able to view their own compliance status, and supervisors and instructors will be able to view the compliance status of their direct reports and/or students.

Please refer to these tip sheets for more information about the portal and how to use it:

For more information about UCLA’s response to the pandemic, please see UCLA’s dedicated COVID-19 website, which includes all campuswide communications on this subject.

We appreciate your continued efforts to remain compliant and keep everyone at UCLA safe.
